Interventional radiology (IVR) and cardiovascular interventional radiology (CVIR) coding is complex and contains a variety of specific coding rules. Traversing the different rules within this specialty can be a challenge; as a result, coding professionals need support to grow their knowledge and expertise when rising to the challenge.

Catheter placement

When catheters are used above the diaphragm, the CPT guidelines state not to use modifier -50 (bilateral procedure). For example, access is gained in the right common femoral artery, advanced to the aorta, and then contrast is injected. The catheter is advanced into the right brachiocephalic artery for injection and contrast with interpretation given. The catheter is then repositioned into the left subclavian artery, where contrast is injected with interpretation provided.

Report CPT codes:

  • 36215 (selective catheter placement, arterial system; each first order thoracic or brachiocephalic branch, within a vascular family) for catheter placement in the right brachiocephalic artery
  • 36215 with modifier -LT (left side) or-59 (distinct procedural service), depending on payer policies, for left subclavian artery..
